Medical Insurance is offered through a Preferred Provider
Organization (PPO) with options of going in or out of network.
In network benefit offers you the freedom to seek treatment from a wide
variety of doctors with a $10 co-pay for physician visits and 80/20 coverage
after deductible (single $200). Should
you choose a physician outside of the network. your co-pay is 50% after deductible.
Dental Coverage is a traditional plan allowing you to
visit any dentist of your choice. Coverage
includes two check-ups and cleanings per year at 100% and covers up to $1000
of treatment per calendar year. Other
dental fees varying % after $50 deductible.
Prescription Drug benefit offers $15/$25/$45 co-pay
for prescriptions or a Mail Order prescription drug option with no co-pay for
maintenance drugs.
Flexible Spending Accounts allow you to pay for certain
Health and Dependent Care expenses with pre-tax dollars.
You select a dollar amount that is deducted from your gross pay, each
pay period, as pre-tax. These dollars
are placed in the Healthcare and/or Dependent Care accounts.
When you incur an expense for medical or childcare, you are reimbursed
from the Flexible Spending Accounts.

Life insurance is provided for covered employees as
part of our health benefit. Optional
add-on life insurance is available for employee purchase, if desired.
Group Long Term Disability insurance is provided for
employees with 5 years full time employment.
